Resident Care Coordinator/RN Opportunity

 

Summary: The Resident Care Coordinator is responsible for ensuring daily organization and delivery of safe, competent, and ethical care in accordance with the Standards of Practice and Code of Ethics of the Nova Scotia College of Nurses.

This position is a supervisory role reporting directly to the Director of Care.

Position Responsibilities:

  • Comprehensive assessment of each resident’s needs, and the development/implementation of individual care plans to meet those needs in collaboration with other nursing staff.
  • Involvement in the introduction and education of nursing staff to Inter RAI at Willow Lodge.
  • Informed communication with all departments.
  • Ensures full completion of fall reports and reportable occurrence forms.
  • Communicates with physicians any resident medical issues requiring attention.
  • Weekly preparation for rounds with Medical Director and other physicians.
  • Communicates with family members any changes in resident’s condition or medications, falls or serious incidents, or consultations with other practitioners.
  • Collaborates with all members of the health care team, including Dietitian, PT/OT assistant, RNs, LPNs and CCAs.
  • Delegates tasks in relation to resident care needs according to scope of practice.
  • Takes the lead in multidisciplinary meetings and resident care conferences.
  • Participates on other committees, such as pharmacy, wound care, OH&S and Infection Control.
  • Supports scheduling when scheduler is not in (except for weekends, holidays and regular after hours, this is supported by the Team Leaders.) This may involve re-assignment of staff according to facility needs.
  • Promotes and respectfully supports the Policies and Procedures of Willow Lodge
  • Participation in annual licensing preparation.
  • Participation in the orientation of new nursing staff, including travel staff nurses, through mentoring, motivating, and communicating job expectations.
  • Mentoring licensed staff to perform skills within their scope of practice that they may not be familiar with.
  • Liaises with local pharmacist for all medication related issues, including new orders, ordering of stock medications etc. This also includes the proper documentation of receiving, counting, and returning all medications to the local pharmacy, including narcotics.
  • Regular diagnostic phlebotomy
  • May be asked to administer medications, or occasionally asked to work as the second nurse if a shift is not filled (very seldom occurs)
  • Documentation following standards of practice guidelines.
  • Take part in resident immunization clinics in Willow Lodge.
  • Adheres to the best practices of Infection Control/ Public Health/Occupational Health
  • Palliative Care support when required.
  • Coordination of Medical and other appointments (dental. Ortho, etc.)
  • Support for occasional coverage for the Director of Care during vacation etc.

CCAs

                                               

Job Summary:

Continuing Care Assistant is responsible for providing direct and indirect Resident care under the supervision of the Charge Nurse and/or Director of Resident Care.  The CCA works within his/her scope of practice and follows established facility policies and procedures while providing care, and coordinates resident care with other departments within Willow Lodge.

 

Responsibilities/Duties:

 

  1. Must have completed Continuing Care Assistant’s Course.
  2. Must embrace the Eden philosophy in alleviating the 3 plagues of loneliness, helplessness and boredom.
  3. Must be physically and mentally suitable for employment.
  4. Must demonstrate effective communication skills – verbal, oral and written.
  5. Must demonstrate initiative in gerontology and self growth in this field.
  6. Must be excited about change & contribute to changing methods of care
  7. Must demonstrate ability to be a team player.
  8. Computer skills an asset or must be willing to learn.
  9. Must accept and use constructive feedback for personal & professional growth.

Environmental Care Partners are responsible for maintaining a high standard of cleanliness and tidiness within Willow Lodge and maintaining an adequate supply of clean linen and clothing.

 

Food Service Care Partner is a team player who displays understanding of the needs of our Residents, while providing a pleasant dining experience.

 

General Qualifications, Knowledge, Education and Skills:

  1. High School diploma desirable or equivalent
  2. Must have WHMIS.
  3. Completion of Alzheimer’s Course an asset.
  4. Must demonstrate a personality compatible to this position with a genuine concern for the elderly and mentally/physically challenged.
  5. Must be in good physical and mental health.
  6. Must demonstrate effective communication skills, oral, verbal and written.
  7. Must demonstrate initiative.
  8. Must be able to work every second weekend.
  9. Experience working with the elderly and mentally challenged adults considered an asset.
  10. Good people skills.

Food Service Knowledge, Education and Skills:

  1. Must have Food Handlers Course.
  2. Must have some experience in food preparation and an understanding of special diets.
